Effective Nov. 23, 2008: All National Change of Address (NCOA) lists will require a Processing Acknowlegement Form (PAF) to be completed by each list owner before we can process any lists for mailing. This form will be required for all mail.

We ask that all our clients who use our mailing services fill out the LIST OWNER section of the form and fax it back to our attention at: (773) 267-6482. We will then use this form when we process your list. Note: this form only needs to be completed once a year.
